Submission of Identity Certificate details(Annexure "B")

Hi,

My wife is a PSU(bank) employee. Per rule of Section 6(2), Annexure "B" can be submitted along with passport application of my wife for no police verification. Can any one suggest me who will be authority(controlling) for a bank branch to provide her the certificate?

It is intimated with a concern that I, Ratnesh Kumar Pandey S/o Sh. Bays Pandey resident of Vill- Ekwari, P.O- Biswar, Via- Mairwa, P.S- Guthani, District- Siwan, Bihar, have applied for the passport on 07/08/2015 ie, file no PA10790508656. On 12/04/2016 when checked by internet, the present position for the concerned P.S, it indicates that the file has been send to Patna passport office
File No.- A10790508656, Name- RATNESH KR PANDEY Receipt No.- 15/38571, Receipt Date
-18-09-2015, P.S. Dispatch No.- 3025, SP Office DR No. 15/41154 SP Office DR Date-7/10/2015, Police Station- GUTHANI, Current Status- Send To Patna
When checked from Patna passport office website it indication that the file is pending with concerned P.S for verification These are contrary reports not understood. Keeping in view the urgency of the matter and the contradicted reports (as enclosed above).
You are request to find the facts and apprise please.
